How do I refer a patient?
Contact our practice by telephone, e-mail or fax. Alternatively complete the online referral form below and press Submit to send it to us. You may also download and print a referral form here. Your patient will be contacted promptly.
Please direct parents to this website for information. If parents have any further questions our friendly receptionist staff would be more than happy to answer them.
Emergencies
Please call our practice on 8021 4242 or 8580 5378 and leave a message stating the nature of the emergency and your contact details. We will endeavour to return your call as soon as possible.
